- Alleged Victim (AV), a resident of the facility and Alleged Perpetrator 2 (AP2), an employee of the facility engaged in a physical altercation, which resulted in AV being injured. On or about October 2, 2023, Alleged Victim (AV) was in the dining room with Witness 4 (W4). Alleged Perpetrator 2 (AP2) approached AV to administer AV's medication that were due at that time. AV refused the medications being provided by AP2. W4 stood between AV and AP2 and encouraged AV to take AV's medications. AV moved W4 due to perceiving that AP2 was a threat to W4. AV reached for AP2 and AP2 batted AV's hands away. AV grabbed AP2 by both arms and pulled AP2 towards AV, which caused both AV and AP2 to lose their balance. AV fell backwards, which caused AP2 to fall on top of AV. AV and AP2 hit the wall on the way down to the floor. At this time, multiple staff responded and separated AV and AP2. During the separation, AP2 did not let go of AV's arm and continue to move in the direction of AV. Facility staff moved AP2 to another area, which was away from AV. AV sustained a small abrasion to AV's right cheek as a result of incident. AP2 physically abused AV by having a physical altercation with AV resulting in AV sustaining an injury. The facility failed to provide a safe environment which is a violation of Oregon Administrative Rules.
