Findings: On or about October 4, 2023, the Alleged Victim (AV) was using the restroom and pressed his/her call light for assistance. A staff member arrived and advised AV they would send someone to assist AV shortly. AV pressed his/her call light multiple times waiting for staff to arrive. After approximately a 45 minute wait, when AV was starting to feel cold and his/her bottom was numb, AV removed him/herself from the toilet and tried to clean him/herself, which he/she felt she didn't do a good job. Later a staff member did show up and assisted AV to clean him/herself better. According to staff, the call light system has been having issues and not always working. The facility's failure to ensure call lights were working properly and being answered timely resulted in unreasonable discomfort for AV. The failure is a violation of resident rights, is considered neglect of care and constitute abuse.
