Findings: The facility failed to appropriate care plan and provide services according to the Alleged Victim (AV)'s needs, relating to falls and lack of appropriate interventions. According to an investigation, AV experienced approximately three non-injury falls, two of which were unwitnessed, between August 3, 2023, and October 1, 2023. On or about October 2, 2023, AV experienced another unwitnessed fall, which resulted in AV sustaining two fractured ribs. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
