Findings: The Alleged Victim (AV) has a diagnosis of dementia and is often confused and needing redirection. AV left the facility on or about August 30, 2023 without supervision and was unable to return to the facility without assistance. On or about September 27, 2023, AV was found by staff across the street in a parking lot of an apartment complex. AV's care plan notes AV is an elopement risk, however, there were no interventions in place to address AV's risk of elopement. The facility's failure to properly care plan to ensure AV's safety is a violation of resident rights, is considered neglect of care and constitutes abuse.
