Findings: According to the documentation, the facility failed to ensure staff were trained appropriately for the proper use of a Hoyer lift. The failure resulted in the Hoyer lift to tip over, causing the Alleged Victim (AV) to fall to the floor and sustaining a fracture to the AV’s lower right leg and tail bone, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
