Findings: The Alleged Victim (AV) is a known fall risk and has suffered multiple falls, however, AV's care plan lists AV as independent in transfers and ambulation. On or about October 7, 2023, AV suffered an unwitnessed fall and was transported to the emergency room where he/she was diagnosed with a fractured hip. The facility's failure to ensure interventions were appropriate and properly care plan to reduce falls is a violation of resident rights, is considered neglect of care and constitutes abuse.
