Findings: Witness #1 (W1) has a history of verbal aggression towards staff and residents. On or about October 7, 2023 and October 9, 2023, W1 became verbally aggressive towards AV, causing AV to be scared and feel unsafe in the facility. The facility failure to keep AV safe is a violation of resident rights, is considered neglect of care and constitutes abuse.
