Findings: On October 7, 2023, facility staff failed to visually observe Allege Victim (AV) take his/her a.m. medications. AV did not take his/her a.m. medications left in a cup in his/her room. Based on interviews and facility documentation, AV's MAR was inaccurately marked, indicating AV's medications were administered both a.m. and p.m. on October 7, 2023. The facility's failure to administer AV's medication as ordered is a violation of Oregon Administrative Rules.
