Findings: Alleged Victim (AV) relies on the facility for his/her care. Between July 5th, through October 15th, 2023, AV had sustained approximately 18 falls, AV suffered injuries during 12 of those falls. On or about October 10, 2023, AV was asleep in a chair in the common area, his/her walker was not in the common area near him/her. AV had visible abrasion injuries to his/her forehead above and including his/her left eye, visible abrasions above and including his/her left eyebrow, the bridge of his/her nose, just below the bridge of his/her nose, and there was a long open wound from the inside corner of his/her left eye, diagonal to his/her upper left forehead. This was all documented in a photograph taken of AV. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP24-00078 $500.00 fine assessed
