Findings: According to the documentation, the Alleged Perpetrator 2 (AP2) failed to follow the Alleged Victim’s (AV) service plan while pushing the AV in their wheel chair. On or about October 06, 2023, a video showed the AP2 pushing the AV in their wheelchair into their room with their arms on the arm rests. The service plan instructs the caregiver to place the AV’s hands and arms on their lap to prevent their arms to bump into objects which could cause bruising or skin tears. The failure resulted in the AV’s arm to scrape against their door causing a skin tear. The AP2 admitted to not reviewing the AV’s care plan, which is a violation of resident rights and is considered abuse. The facility failed to provide a safe environment which is a violation of Oregon Administrative Rules.
