Findings: Alleged Victim’s (AV’s) care plan states AV will go to the double doors and push on them; staff are to walk around the community with AV. On or about October 14, 2023, AV stepped out of the building when AV saw an opportunity. As a result, AV eloped from the secure unit of the memory care facility. AV was found outside the front door of the assisted living facility. The facility failed to follow the care plan by not having a staff member walk around AV putting AV in risk of harm, which resulted in AV eloping, which is ai violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP24-00438 $375.00 fine assessed
