Findings: Witness #1 (W1) is known to engage in altercations. On or about October 16, 2023, W1 was walking down the hall and slapped the Alleged Victim (AV) on the side of AV's forehead leaving a red mark and agitating AV. The facility's failure to have interventions in place to reduce W1 altercations placed AV at risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP24-01019 $375.00 fine assessed
