Findings: “Alleged Victim’s (AV’s) service plan states he/she is dependent on his/her metabolic medication to manage his/her metabolic disease. On or about October 22, 2023, AV sustained a fall due to AV having low metabolic markers. As a result of the fall, AV sustained bruising to his/her head, a brain bleed, and a skin tear to his/her elbow. At the time staff discovered the injuries, AV’s metabolic marker was at 44. AV suffered three (3) falls related to low metabolic markers prior to the fall on October 22, 2023. Even after all of the falls, AV’s service plan does not appear to be updated to better reflect a person-centered intervention to manage AV’s metabolic disease, which resulted in AV sustaining falls, which is a violation of resident rights, is neglect of care and constitutes abuse.”
