Findings: The Alleged Victim (AV) has suffered multiple falls with and without injury. AV was not care planned as a fall risk, even though AV had suffered multiple falls. AV suffered an injury fall on August 16, 2023 where he/she suffered an abrasion. The facility failed to properly are plan as it relates to falls, placing AV at risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
