Findings: Alleged Victim’s (AV’s) care plan states he/she experiences bowel or bladder incontinence and requires a schedule toileting plan or uses a call light for assistance throughout the day. AV requires assistance with transfer/ambulating and requires hands-on or stand-by assistance. On or about October 24, 2023, AV called for assistance getting to the bathroom with no response. AV called the front desk and was told someone would be right up. 30 minutes later AV called the desk again and got the same answer. Two hours and a half later a caregiver answered the call light. By this time, AV had urinated on his/herself and his/her chair and felt humiliated. AV needed his/her clothes to be changed entirely. The facility to follow AV’s care plan around toileting assistance and failed to respond to the call light in a timely manner, which is a violation of resident rights, is neglect of care and constitutes abuse.
