Findings: The Alleged Victim (AV) had two recent falls, on October 20, 2023 and October 26, 2023, both falls were noted to be without injury. On or about October 30, 2023, AV was found to have a bruise on his/her left eye. No interventions were put into place following the falls. AV had started new medication and was found that this medication was causing lethargy and weakness. The medication was discontinued on October 27, 2023. The facility's failure to implement interventions related to fall and to provide a safe environment for AV is a violation of resident rights, is considered neglect of care and constitutes abuse.
