Findings: On or about October 7, 2023, the Alleged Victim (AV) suffered a fall in his/her room. Alleged Perpetrator #2 (AP2) and another staff member assisted in getting AV up from the floor. AP2 was in a hurry and pulled on AV in a rough manner causing pain to AV. AP2 wasn't following instructions from the other staff member to slow down. AP2's actions caused pain to AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse. The facility's failure is a violation of Oregon Administrative Rules.
