- Findings
- Alleged Victim (AV) and Witness 1 (W1) have a history of resident-to-resident incidents with others and a history of altercations between each other. AV has impulsive behaviors that may agitate other facility residents and facility staff are to use interventions such as: talking in a calm voice, offering to sit and have coffee with AV, offer to walk with AV, talk about [named] country, and offer for AV to talk on the phone with AV's relative. Staff are to monitor AV for seeking out W1 and redirect AV to other areas of the facility away from W1. W1 was moved to a different part the facility due to W1's behaviors on October 25, 2023. Facility staff are to monitor W1 for increased aggressive behaviors/agitation when around other residents. Facility staff are to encourage W1 to go to W1's neighborhood and listen to quiet/soft music and offer W1 a snack such as chocolate chips. On or about October 29, 2023 an altercation between AV and W1 took place in AV’s neighborhood. Staff did not redirect W1 back to his/her neighborhood prior to the incident. W1 hit AV, staff then redirected W1 and AV away from each other. AV was angry and upset after the incident. The facility failed to follow the care plan, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
