Findings: The Alleged Victim (AV) relies on the facility for his/her care. AV and W1 have aggressive behaviors and W1 is care planned for W1 to be separated from AV. According to an investigation, on or about October 26, 2023, AV coming down the hallway when W1 from punched AV in the face, resulting in a bruise and small lacerations on his/her face. The facility failed to provide a safe environment and follow W1’s care plan, which is a violation of resident rights, is neglect of care which constitutes abuse.
Sanction: RCFCP25-00552 $250.00 fine assessed
