Findings: The facility failed to provide appropriate services according to resident needs, as it relates to care planning and call light. According to an investigation, AV experienced approximately four falls between September 3, 2023, and October 31, 2023, which resulted in repeated unreasonable discomfort. During the fall on or about October 31, 2023, AV pushed the call button and received no response from care staff, so he/she attempted to go to the restroom on his/her own. According to the staff, AV’s call light had not been working for approximately two weeks. The facilities failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
