Findings: On or about October 29, 2023 during med count reconciliation, it was discovered that several medications belonging to the Alleged Victim (AV) were discovered missing. Alleged Perpetrator #2 (AP2) had worked the night shift prior to the medications coming up missing. AP2 refused a drug test and refused to complete the count on the shift change in question. AP2's actions are a violation of resident rights, are considered neglect of care and constitute financial abuse. The facility's failure is a violation of Oregon Administrative Rules.
