Findings: The Alleged Victim (AV) and Witness #1 (W1) were known to be involved in altercations with each other, and were known to have at least two previous altercations. On or about November 1, 2023, AV and W1 engaged in an altercation where W1 was holding AV's arm and then punched AV in the jaw three times. AV was noted to have redness to his/her jaw after the interaction. The facility's failure to implement meaningful interventions to reduce altercations placed AV at risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
