According to the investigation, the facility failed to properly care plan to mitigate risk of falls and self-transferring. AV had approximately five falls between September 1, 2023, and October 19, 2023, resulting in repeated unreasonable discomfort. This failure is a violation of resident rights, is neglect of care which constitutes abuse.
