Findings: The Alleged Victim (AV) was a known fall risk, had a history of falls. According to an investigation, on or about November 4, 2023, the AV was found on the floor of their room with a cut to the nose requiring three stiches and fractures to the nose and maxillary bone. The facility failed to provide appropriate services according to AV's needs, relating to care plans and lack of appropriate interventions and instructions, to mitigate the risk of injury due to AV's falls, which is a violation of resident rights, is neglect of care, and constitutes abuse.
