Findings: Alleged Victims (AV’s) care plan indicated AV requires two-person assistance for transfers. On or about October 29, 2023 AP2 was transferring AV by themselves when AV bumped their face on AP2’s shoulder, causing a bruise on the right side of AV’s lip. The failed to properly train or provide documentation indicating that AP2 received training on resident transfers. AP2 failed to follow the care plan.
