Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about November 06, 2023, AV experienced a bout of diarrhea and pushed h/h call light for hands on assistance with cleaning and getting off the toilet. According to the facility call light log, AV pulled h/h call light twice, waiting 3hrs, 44mins, 56sec and 4hrs, 34mins, 43sec. AV attempted to clean him/her self-up but did not feel s/he did an adequate cleaning resulting in loss of personal dignity. While waiting for staff to assist AV off the toilet, AV's legs went numb causing unreasonable discomfort. The facility failed to answer call light in a timely manner, which is a violation of resident rights, is neglect of care and constitutes abuse.
