Findings: Witness 1 (W1) has a known history of resident-to-resident altercations. W1’s care plan states he/she is to always have a one-to-one when W1 is awake. On or about October 28, 2024, W1 entered Alleged Victim’s (AV’s) room, which resulted in AV not leaving and a physical altercation taking place between W1 and AV. W1 did not have a one-on-one staff member present as stated in the care plan, which could have prevented W1 from entering the wrong room and would have prevented the altercation. The facility failed to assure the care plan was being followed, which is a violation of resident rights, is neglect of care and constitutes abuse.
