Findings: The Alleged Victim (AV) is service planned for cognitive deficits and interventions include safety checks. AV has a history of leaving the facility without assistance. On or about November 7, 2023, AV left the facility without assistance and was found almost a half mile from the facility and was returned to the facility by law enforcement. The facility's failure to provide a safe environment placed AV at risk for serious harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
