Findings: The Alleged Victim (AV) relies on the facility for care and has a history of falls. According to an investigation, on or about November 11, 2023, AV had an unwitnessed fall. AV hit their head and leg and were transported to the hospital for treatment. The facility failed to implement interventions to mitigate AV’s increasing fall risk, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
