Findings: The Alleged Victim (AV) was a known fall risk and care planned for an alarm to be placed when AV is sitting in recliner and wheelchair. The facility failed to follow AV’s care plan. According to an investigation, AV was placed in his/her recliner without the alarm. AV attempted to get out of his/her recliner and fell, which resulted in AV hitting his/her head, requiring liquid stitches. This failure is a violation of resident rights, is neglect of care which constitutes abuse.
