Findings: The facility failed to provide assistance with bathing to the Alleged Victim (AV). Approximately in May 2023 there is only documentation of staff showering AV two times, and approximately in August 2023 there is no documentation of AV being showered between 08/03/23 and 08/28/23 resulting in AV experiencing unreasonable discomfort, and redness on h/h head from itching. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
