Findings: Alleged Victim (AV) relies on the facility for his/her care and has a known history of falls. Staff were directed to monitor AV anytime they were up walking and to ensure AV has on appropriate footwear. On or about November 17, 2023 AV had an unwitnessed fall in the dining room and when found AV complained of pain. AV was barefoot and had just walked across wet carpets that had just been cleaned. AV was transported to the hospital for evaluation to ensure that the head injury sustained two days prior had not worsened. The facility failed to implement interventions and / or follow AV care plan, which is a violation of resident’s rights is neglect of care and constitutes abuse.
Sanction: RCFCP24-00395 $1125.00 fine assessed
