Findings: Witness #1 (W1) has documented acts of aggressive behavior with staff, other residents, and resident-to-resident altercations from approximately October 11, 2023, to November 15, 2023. W1’s care plan does not include information about W1's aggressive behaviors and does not list instructions or interventions for staff, related to W1's aggressive behaviors. On or about November 16, 2023, Alleged Victim (AV) was walking the hall and W1 was roaming the building in h/h wheelchair. W1 started yelling at AV, AV stopped to talk to W1, W1 hit AV's hand and continued to swing at AV. W1 grabbed AV's sweater causing AV to lose h/h balance and fall over. AV hit the crown of h/h head on the handrail resulting in AV reporting pain. EMS was called, AV did not need to be transported, but did have a bruise on h/h head.
