Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about November 19, 2023, AV was discovered by staff with redness to their cheek, bloody lower lip and discoloration to their left eye and left brow. Upon review of camera footage, it was determined that AV was the victim of a resident-to-resident altercation in which Witness 1 (W1) attacked AV, punching AV several times in the face and left arm while AV was standing in the hallway. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
