Findings: The Alleged Victim (AV) is known to yell and become belligerent in the common area. Witness #1 IW1) has become irritated and hit AV before during these outbursts. On or about November 14, 2023, AV was having behaviors and yelling in the common area. AV walked towards W1's door to throw something in the trash. W1 opened his/her door and told AV to shut up and slapped AV in the face. The facility's failure to ensure resident safety by not having appropriate interventions in place to reduce the risk of altercations is a violation of resident rights, is considered neglect of care and constitutes abuse.
