Findings: The facility failed to provide appropriate services according to the Alleged Victim (AV)'s needs, the facility failed to implement a service plan and interventions to address AV frequent falls. On or about November 23, 2024, AV went to the hospital due to AV having an altered mental status, right-sided weakness, slurred speech, and abnormal behavior. Upon examination, AV had healing bruises all over h/h lower legs, excoriation on nose and legs, dirt under h/h nails and AV's right hip was dislocated. The hospital attempts to relocate AV's hip was unsuccessful due to the stage of injury and healing. AV experienced pain from persistent hip dislocation and contribute to AV"s agitation and behaviors, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP24-01193 $1500.00 fine assessed
