Findings: According to the documentation, the facility failed to properly care plan for the Alleged Victim’s (AV) known behavior of wandering into resident rooms. The failure resulted in the AV to wander into Witness 1’s room (W1) upsetting them causing an altercation between the two residents. The AV sustained a bump on their eyebrow and a bruise on their left eye, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
