Findings: On or about November 25, 2023, the Alleged Victim (AV) was found on the floor by another resident, who called emergency services for assistance. It is not known for sure how long AV was on the floor prior to emergency services were called. Staff were not available at the time of the fall, as one was on break and the other was downstairs doing laundry. At least one staff was to be present on the floor at all times, which was not followed. Alleged Perpetrator #2's involvement in this case has been determined to be inconclusive. AV's fall and trip to the emergency room resulted in AV having hip pain. The facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
