Findings: According to the documentation, the facility failed to provide a safe environment for the Alleged Victim (AV) by not implementing appropriate interventions for Witness 1’s (W1) known behaviors with residents. On or about November 23, 2023, W1 approached the AV and started yelling profanities at the AV. W1 then grabbed the AV’s walker and while attempting to leave, the AV fell to the ground, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
