Findings: The Alleged Victim (AV) relies on the facility for his/her care and is documented as a fall risk. According to an investigation, between approximately April 15, 2023 and October 22, 2023, AV was documented to have fallen or have been found on the floor 18 times and AV sustained injury or reported significant pain in approximately ten (10) of the incidents. On or about November 24, 2023, AV was again found on the ground in their room, placing AV at risk of harm. The facility failed to implement effective interventions to mitigate AV’s increasing fall risk and provide a safe environment for AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
