Findings: Witness 1 (W1) has a known history of sexual behaviors. On or about November 14, 2023, W1 was found touching an opposite gender resident in an inappropriate sexual manner. On or about November 22, 2023, W1 was found in the common area with his/her hand down Alleged Victim’s (AV’s) shirt. When the video footage was checked, staff realized that W1 did the same thing to another resident as well. The facility failed to provide a safe environment for AV, putting AV in risk of harm, which is a violation of resident rights, is neglect of care and constitutes abuse.
