Findings: Alleged Victim’s (AV’s) care plan states he/she can ambulate independently and can transfer in and out of bed. It also states he/she is independent with toileting. Between August 25th, through December 4, 2023, AV sustained multiple falls. Some of the falls occur while AV was self-toileting and in one of the falls while self-toileting, AV sustained a laceration which required AV to receive 16 staples. The facility failed to care plan arounds AV, known history of falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP24-00702 $1500.00 fine assessed
