Findings: According to the documentation, the facility failed to properly care plan appropriate fall interventions for the Alleged Victim’s (AV) history of falls. On or about October 14, 2023, the AV was attempting to self-transfer into their recliner and was found lying on the floor by staff. The AV complained of shoulder pain, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
