Findings: According to the documentation, the facility failed to train staff appropriately for 1:1 care for the Alleged Victim (AV). On or about November 5, 2023, AV’s 1:1 caregiver left the AV to assist another caregiver with another resident. The AV was found lying on the floor in their room due to an unwitnessed fall, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP24-00543 $2000.00 fine assessed
