Findings: On or about December 8, 2023, Alleged Victim (AV) brought a chair to the front gate of the facility and eloped over the fence. Based on facility documentation, AV was very new to the facility and staff were unfamiliar with AV. However, AV was able to move a piece of furniture to the outside of the facility and use the piece of furniture to climb over the gate without staff noticing AV's exit seeking behavior. AV was found approximately 30 minutes after AV was noticed missing, unharmed. The facility failed to provide a safe environment which is a violation of Oregon Administrative Rules.
