Findings: The Alleged Victim (AV) relies on the facility for his/her care, requires staff assistance with transfers and has a history of falls. On or about December 8, 2023, AV was found on the floor after apparently attempting to self-transfer. AV was taken to the hospital where AV was diagnosed with a closed head injury. The facility failed to appropriately care plan and implement reasonable interventions to assure AV’s safety and address AV’s ongoing falls, which is violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP24-00452 $1125.00 fine assessed
