Findings: The Alleged Victim (AV) is a known fall risk with a history of falls. According to an investigation, on or about November 9, 2023, AV experienced a fall and was found on the floor resulting in a large lump near AV's spine between shoulder blades. The facility failed to appropriately care plan and effectively implement interventions to address AV's ongoing falls, which is a violation of resident's rights, is considered neglect of care and constitutes abuse.
