Findings: The facility failed to provide a safe environment to the Alleged Victim (AV). On or about December 14, 2023, AV was found outside the facility, without staff knowledge, resulting in a head laceration. AV was sent to the Emergency Department for treatment. AV experienced pain and unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
