Findings: Witness 1 (W1) is known to get into altercations and have aggressive behaviors. W1 is known to wander throughout the facility. On December 15, 2023, W1 entered Alleged Victim’s (AV’s) room thinking it was h/h room. AV then told W1 that this was h/h room and W1 struck AV. AV then struck W1. As a result of the altercation, AV had scratches to h/h nose and W1 had bruising to each cheekbone and a bleeding lip. W1 has previously caused injury to other residents as a result of physical resident-to-resident altercations. Interventions in place to reduce W1’s behaviors are staff to offer to walk with W1, staff to offer W1 a snack, and staff to sit with W1 in h/h room to minimize distractions. There are no interventions in place to address W1’s wandering into other resident rooms. The facility failed to properly plan care for W1 leading to continued aggressive behavior and wandering into other resident rooms, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
