Findings: The Alleged Victim (AV) relies on the facility for his/her care and is a known fall risk. According to an investigation, on or about December 18, 2023, AV had and unwitnessed fall and was on the floor for an undisclosed amount of time. The facility failed to implement fall prevention interventions commensurate with the frequency and severity of AV's known falls and failed to follow interventions already in place resulting in unreasonable discomfort. The facility failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
